Our Integrated Community Network (ICN) spans the entire state and is comprised of 38 Skilled Nursing Facilities (SNF) and 1 Home Health Agency (HHA) designated as Tier 1 partners, and 24 SNFs and 10 HHAs designated as Tier 2 partners. These settings have been vetted for quality, engagement, and network citizenship, which is reviewed on a regular basis.
