Pharmacy Services

At ICP, pharmacists collaborate with our members to improve pharmacy quality measures in value-based contracts.  Our pharmacists work with patients and providers to improve medication management, help reduce medication-related problems such as adverse drug events and polypharmacy, and deliver direct patient care through home-visitations for high-modifiable-risk patients.  The pharmacy team is responsible for overall pharmacy population health management and pharmaceutical education for the network.  They provide feedback to prescribers on pharmaceutical trends, cost-saving initiatives, and overall prescribing performance.

ICP has partnered with the University of Connecticut School of Pharmacy and serves as a training site for Doctor of Pharmacy candidates.  Led by faculty members Jeffery and Polomoff, students contribute to ICP’s value and quality initiatives by addressing medication adherence gaps, developing educational materials and collaborating with providers.
