Pharmacy Services

At ICP, pharmacists collaborate with our members to improve pharmacy quality measures in value-based contracts. Our pharmacists work with patients and providers to improve medication management, help reduce medication-related problems such as adverse drug events and polypharmacy, and deliver direct patient care for high-modifiable risk patients. The pharmacy team is responsible for overall pharmacy population health management and pharmaceutical education for the network. They provide feedback to prescribers on pharmaceutical trends, cost-saving initiatives, and overall prescribing performance.

ICP has partnered with the University of Connecticut School of Pharmacy and serves as a training site for Doctor of Pharmacy candidates. Led by faculty members Jeffery and Polomoff, students contribute to ICP’s value and quality initiatives by addressing medication adherence gaps, developing educational materials and collaborating with providers.

Director of Clinical Pharmacy Services

Dr. Sean M. Jeffery is Director of Clinical Pharmacy Services at Integrated Care Partners, the care coordination and clinical integration arm of Hartford HealthCare Group. Within this accountable care organization, Dr. Jeffery is responsible for pharmacy network development and strategy, population health management, and support of an integrated care-management team.

Population Health Clinical Pharmacist

Dr. Christina Polomoff is a Population Health Clinical Pharmacist at Integrated Care Partners. She supports the care management team by providing medication therapy management consults and direct patient care for high-modifiable risk patients. She also develops and implements population health management strategies that focus on network pharmacy performance metrics. In addition, she serves as an Associate Clinical Professor as part of her joint position with the University of Connecticut School of Pharmacy.
