Community Care Management

ICP provides assistance by improving patient outcomes through care coordination and an enhanced focus on quality and health. Collaboration with Community Care Managers improves the outcomes of high-risk patients as well as their overall health and quality of life, while reducing high-cost episodes of care.


Pharmacy Services

Our pharmacy services focus on pharmacy network development and strategy, population health management and direct patient care through home-visitations for high-modifiable-risk patients.

    Epic, available at a significantly reduced cost, offers a uniform electronic health record system across Hartford HealthCare to better coordinate patient care and allow ease of reporting on quality performance measures.

  • MACRA/MIPS

    To potentially earn a positive payment adjustment under MIPS, you must be ready to report data about the care you provided and how your practice used technology in 2017 to CMS by the deadline, March 31, 2018. Click here to learn more.
