ICP has created a practice transformation specialist position to support our adult primary care community practices and assist with their quality metrics and financial rewards.
Practice Transformation Specialists will support practices in the following ways:
- Identify barriers and process improvement opportunities to close Gaps-in-Care
- Collaborate with practice leads to execute a quality improvement plan on key metrics
- Support physicians in understanding scorecards and using their performance data
- Educate and be a resource for questions and concerns related to clinical initiatives and outcomes
- Assist practices with efficient streamlined workflows and required documentation
Engaging with our Practice Transformation Specialists allows our physician members to achieve greater success under new value-based payment models. By working together to improve the quality and reduce the total cost of patient care, members are rewarded through earned incentives, from Integrated Care Partner’s commercial and government contracts.
Practice Transformation Specialists will work in close collaboration with your ICP Provider Relations Specialist and the ICP Director of Population Health to support your physicians and staff/practice leads.
