
Dr. Christina Polomoff is a Population Health Clinical Pharmacist at Integrated Care Partners. She supports the care management team by providing medication therapy management consults and direct patient care for high-modifiable risk patients. She also develops and implements population health management strategies that focus on network pharmacy performance metrics. In addition, she serves as an Associate Clinical Professor as part of her joint position with the University of Connecticut School of Pharmacy.
Dr. Polomoff has an active leadership role in the American Society of Consultant Pharmacists (ASCP) as Director-at-large nationally for the 2022 – 2025 term. She co-chaired ASCP’s National Medication Safety and Transitions of Care Committee 2019 – 2022, and served as interim President for the ASCP Connecticut Chapter in 2022. Dr. Polomoff also serves on the Board of Directors for the Connecticut Pharmacists Association as First Vice President, and holds an appointment on the State of Connecticut Department of Development Services Institutional Review Board.
Dr. Polomoff graduated from the University of Connecticut with her Doctor of Pharmacy and Business Minor. She completed a Post Graduate Year-1 residency at the VA Connecticut Healthcare System followed by a Post Graduate Year-2 ambulatory care residency at The Johns Hopkins Hospital. She is board certified in both ambulatory care and geriatrics. Dr. Polomoff is a fellow for the American Society of Consultant Pharmacists.
