Hartford HealthCare, Connecticut’s most comprehensive health system, and Tufts Health Plan, one of the country’s highest rated nonprofit health plans, are forming a company to be known as CarePartners of Connecticut, a new health plan for Connecticut Medicare-eligible individuals.  This is the first collaboration of its kind in Connecticut. 

Seniors with Medicare or those Medicare-eligible will soon have a new choice for health insurance coverage.  CarePartners of Connecticut combines the strengths of two organizations recognized for high quality with the common mission to improve the health and wellness of their communities. 

Care models and programs to increase access to care and services and improve the coordination of care for those who need it most will be developed collaboratively by Hartford HealthCare and Tufts Health Plan. Data and information will support providers and care managers with actionable information that will help to identify where gaps in care exist.

CarePartners of Connecticut expects to begin covering members on January 1, 2019, pending regulatory approval.     

A statewide network of Connecticut providers is being established, that in conjunction with Hartford Healthcare will provide services to enrollees.
